Studies that can be performed at Midwest Sleep Institute
Diagnostic Polysomnography: |
6 hours sleep study starts at 9 pm and ends at 5 am to diagnose Sleep Disorder Breathing and Periodic Limb Movement disorder. |
CPAP Titration Study: |
6 hours sleep study starts at 9 pm and ends at 5 am to treat obstructive sleep apnea. |
| Split Night Sleep Study: |
6 hours sleep study to diagnose and treat Sleep Apnea. Only for Severe patients. |
Multiple Sleep Latency Test: |
A series of five naps during day time todiagnose narcolepsy. This is usually preceded by diagnostic polysomography. |
Procedure for referring Patient:
- Please determine what study is needed.
- Please download and fill up the referral form below with the insurance information.
- Fax the form to our office at 847-855-8990.
- Our office will contact the patient and your office to confirm the date of appointment.
- Please mention on the form if study will be read by the doctor themselves.

Once you fax the order, our office will confirm patient eligibility and call your office and the patient. Patient will be responsible for all the copay and the deductibles.
